All Sports Physicals must be completed, dated, signed, and stamped by your doctor on the official New York State Physical Form within one year from the starting date of the desired sport (see www.hwbulldogs.com for sample). All physical examinations for school are to be documented on the NYS Required Health Examination Form. No other physical examination form will be accepted. Physical forms must be filled in completely and cannot include “see attached.” Only immunization records may be attached to physical forms. The examination form needs to indicate that the student athlete can participate in all activities without restriction including the cardiac history review. The health examination for qualifying a student's participation in athletics is valid for a period of 12 continuous months - through the last day of the month in which the exam was conducted; and through the entire sports season, even if the exam’s expiration is before the season is completed. All forms must be uploaded to Family ID. The form can be accessed here.
Please contact your building nurse via email if you have any health/medical related questions. For athletic questions, please contact the athletic office or email the
